How to complete your paperwork:  

• The best way to fill out paperwork is to open the .pdf in Adobe Acrobat, (a free app) and select “Fill & Sign” to complete your paperwork. 
• Alternatively, you can print your paperwork, complete it, and bring to your next appointment. 

Helpful hints:

• We cannot hold your appointment without a deposit
• New patient paperwork is updated every year
• We verify and update insurance information on every visit; be sure to bring your insurance card and a photo ID to every appointment
